Informed Consent, Acknowledgment and Waiver
*
Voluntary Participation: I consent for my child to participate in personal training sessions provided by the trainer listed above. I understand that participation in physical exercise and athletic activities involves inherent risks, including but not limited to muscle soreness, sprains, injuries, and, in rare cases, more serious health concerns.
Health Assurance: I confirm that my child is in good health and has been cleared by a medical professional to participate in personal training sessions.
Liability Waiver: I understand that the trainer and business are not liable for any injury or health condition that may occur during training sessions, except in cases of gross negligence or willful misconduct.
Emergency Authorization: In the event of an emergency, I authorize the trainer to seek medical treatment for my child. I agree to be responsible for any costs incurred.
Media Release: (Optional) I give permission for my child to be photographed or recorded during training sessions for promotional purposes.
Cancellation Policy: I understand and agree to the cancellation policy set forth by the trainer/business, including any applicable fees for missed sessions without proper notice.
LIABILITY WAIVER I UNDERSTAND THAT LIVVY CHOO FITNESS INCLUDES EXPLOSIVE PHYSICAL MOVEMENTS AS WELL AS AN OPPORTUNITY FOR RELAXATION, STRESS RE¬EDUCATION AND RELIEF OF MUSCULAR TENSION. AS IS THE CASE WITH ANY PHYSICAL ACTIVITY, THE RISK OF INJURY, EVEN SERIOUS OR DISABLING, IS ALWAYS PRESENT AND CANNOT BE ENTIRELY ELIMINATED. IF I EXPERIENCE ANY PAIN OR DISCOMFORT, I WILL LISTEN TO MY BODY, DISCONTINUE THE ACTIVITY, AND ASK FOR SUPPORT FROM THE INSTRUCTOR. I WILL CONTINUE TO BREATHE SMOOTHLY AND MODIFY THE EXERCISES. I ASSUME FULL RESPONSIBILITY FOR ANY AND ALL DAMAGES, WHICH MAY INCUR THROUGH PARTICIPATION. THE FITNESS WORKOUT IS NOT A SUBSTITUTE FOR MEDICAL ATTENTION, EXAMINATION, DIAGNOSIS OR TREATMENT. LIVVY CHOO FITNESS WORKOUT IS NOT RECOMMENDED AND IS NOT SAFE UNDER CERTAIN MEDICAL CONDITIONS. BY AGREEING, I AFFIRM THAT I AM IN GOOD HEALTH AND PHYSICAL CONDITION TO PARTICIPATE IN SUCH A FITNESS PROGRAM. IN ADDITION, I WILL MAKE THE INSTRUCTOR AWARE OF ANY MEDICAL CONDITIONS OR PHYSICAL LIMITATIONS BEFORE CLASS. MY AGREEMENT VERIFIES THAT I HAVE MY PHYSICIAN'S APPROVAL TO PARTICIPATE. I ALSO AFFIRM THAT I ALONE AM RESPONSIBLE TO DECIDE WHETHER TO PRACTICE LIVVY CHOO FITNESS WORKOUT AND PARTICIPATION IS AT MY OWN RISK. I HEREBY AGREE TO IRREVOCABLY RELEASE AND WAIVE ANY CLAIMS THAT I HAVE NOW OR MAY HAVE HEREAFTER AGAINST LIVVY CHOO FITNESS AND IT'S INSTRUCTORS * I have read and fully understand and agree to the above terms of this Liability Waiver Agreement.
