The Personal Trainer agrees to provide fitness training services to the Client. These services include but are not limited to personal training sessions, workout planning, nutritional guidance, and fitness assessments. Number of Weekly Sessions: 3 Session Duration: 12

PERSONAL TRAINING Fees and Payment Terms The Client agrees to pay the Personal Trainer the following fees: $150.00 for 12 sessions Payment is due on the first session

Cancellation and Rescheduling Notice: Must notify within at least 2 hours before scheduled session. You are allowed TWO missed sessions per 12 session pacakage. All other missed sessions will count as attended sessions. Late Cancellation Will count as an attended session

THE TRAINER'S LATE POLICY: ANY SESSION THE TRAINER IS MORE THAN 15 MINUTES LATE, THAT SESSION WILL BE FREE OF CHARGE

Termination of Agreement Either party may terminate this contract with written notice. Any unused sessions will be refunded if terminated by trainer. However, unused sessions will not be refunded if terminated by trainee.
